The Capnostream bedside monitor from Medtronic combines Microstream capnography (end-tidal CO2 and respiration rate) with Nellcor pulse oximetry (SpO2 and pulse rate). It also displays the Integrated Pulmonary Index (IPI), a single value summarising a patient's respiratory status.

The Capnostream is a bedside respiratory monitor from Medtronic. It brings two complementary measurements together on one screen: capnography, which reflects ventilation, and pulse oximetry, which reflects oxygenation.
What does the Capnostream monitor?
- Microstream capnography — continuous end-tidal CO2 (EtCO2) and respiration rate, using low-flow sampling lines suitable for intubated and non-intubated patients
- Nellcor pulse oximetry — SpO2 and pulse rate
- Integrated Pulmonary Index (IPI) — a single number, derived from EtCO2, respiration rate, SpO2 and pulse rate, that gives an at-a-glance indication of the patient's overall respiratory status
Why monitor ventilation as well as oxygenation?
Pulse oximetry can lag behind a developing ventilation problem, particularly when a patient is receiving supplemental oxygen. Capnography responds more quickly to hypoventilation and apnoea, so the two measurements together provide earlier warning of respiratory compromise.

- Procedural sedation areas — endoscopy, cath lab, radiology, dental
- Post-anaesthesia care and general wards for patients on opioid analgesia (including patient-controlled analgesia)
- Any setting where patients are at risk of respiratory depression
Clinical context
The Capnostream supports monitoring and clinical judgement; alarms and the IPI are prompts for assessment, not diagnoses. Configure alarm limits for the patient and follow your local sedation and post-operative monitoring protocols.

What is the Integrated Pulmonary Index (IPI)?
A single value calculated from four parameters — end-tidal CO2, respiration rate, SpO2 and pulse rate — that gives clinicians an at-a-glance summary of a patient's respiratory status. It is an aid to assessment, not a diagnosis.
Can capnography be used on patients who are not intubated?
Yes. Microstream sampling lines are available for non-intubated patients, for example during procedural sedation or on the ward, as well as for intubated patients.
Why not rely on pulse oximetry alone?
Oxygen saturation can stay normal for a time during hypoventilation, especially with supplemental oxygen. Capnography reflects ventilation and responds faster to apnoea and hypoventilation, so the two together give earlier warning.
Which patients benefit most from Capnostream monitoring?
Patients at risk of respiratory depression — during and after procedural sedation, after anaesthesia, and on opioid analgesia including patient-controlled analgesia.
